(initial) I am over the age of eighteen (18) and am interested in helping Sweet Souls Foundation
as a volunteer foster parent.
(initial) All of my cats in my home are current on their FVRCP vaccinations.
(initial) I understand that all medical or veterinary procedures considered for my foster pet must
be authorized by a Sweet Souls Foundation administrator - either Kathleen Hom or Sharyl McBride -
prior to authorizing a veterinarian to perform the considered procedures. The request for authorization
must be submitted in writing to one of the administrators. Approval or denial will be remitted in writing
by an administrator to the foster. Failure to submit a request for approval or undertaking a medical
procedure without express approval from an administrator will result in the foster assuming the
responsibility of payment of all expenses incurred.
(initial) I will provide a humane environment (adequate in size and free from hazards, INDOOR
ONLY FOR CATS), loving attention, food, litter and proper care for my foster animal. Food and litter will
be at the expense of the foster. SSF does not reimburse for food and litter expenses.
(initial) I will transport my foster animal to a veterinarian when necessary and after obtaining
approval from a SSF administrator. Gas expenses incurred for veterinary visits, adoption events, etc. will
be the foster guardian’s responsibility to fund and will not be reimbursed by Sweet Souls Foundation.
(initial) I will isolate my foster from other animals in the household for a minimum of 10 to 14
days to ensure the health of my own animals in the home. I understand that this isolation period is
crucial and extremely necessary to prevent the potential spread of contagions the foster kitty may have
acquired while homeless. During this isolation period and if my foster is healthy and not exhibiting signs
of upper respiratory infection (URI), I will request authorization from an administrator to make and
attend an appointment for my foster cat to be snap tested for FeLV/FIV. If my foster has not been
sterilized but has been vaccinated, I will request authorization from an administrator to make and
attend an appointment for my foster to be spayed or neutered. If my foster kitty has not been
vaccinated or is a kitten and has not received their first two of three FVRCP vaccines, I will request
authorization from an administrator to make and attend an appointment for my foster cat to be
(initial) I understand that Sweet Souls Foundation foster cats will be vaccinated with the FVRCP
vaccine before they are considered ready for adoption. I understand that if I am fostering a kitten, they
must receive three FVRCP vaccines and it is my responsibility as their foster to obtain the vaccines
before they are considered adoptable. I understand I will be reimbursed for vaccine expenses up to $15
per vaccine upon submission of the SSF Reimbursement Form and accompanying invoices and
(initial) I understand that all SSF fosters must be sterilized before they are considered adoptable.
It will be my responsibility as the foster guardian, to ensure they are spayed or neutered at a low cost
clinic or vet within 30 days of rescue (excluding pregnant, elderly, or sick animals) and that I will provide
a copy of the spay/neuter certificate to Kathleen Hom. I understand I will be reimbursed for spay/neuter
expenses up to $75 per sterilization procedure upon submission of the SSF Reimbursement Form and
accompanying invoices and documentation.
(initial) I understand that SSF requires all foster guardians to have their foster kitties “snapped”
via FeLV/FIV snap testing before they can be considered adoptable by SSF standards. I understand I will
be reimbursed for snap testing expenses up to $50 per test upon submission of the SSF Reimbursement
Form and accompanying invoices and documentation
(initial) I understand that if my foster cat tests positive for either FeLV or FIV, I will notify a SSF
administrator immediately and a plan of action will be considered and implemented on a case by case
(initial) I understand that there may be other health requirements needed to ensure my foster is
in excellent health and ready for adoption. These may include deworming, flea treatment, ear mite
treatment, ringworm treatment, URI treatment, etc. I understand that I must inform an administrator of
any suspected issues of this nature and will either be advised to obtain the necessary treatment from
the SSF foster online storefront - either for free or for a small fee - or to take my foster in to see a low
cost veterinary hospital or clinic of my choice.
(initial) I understand that on occasion there are animals that come to us with more extreme
health issues than those known to the Sweet Souls team prior to their rescue and that sometimes these
issues require more extensive veterinary care, may be life-threatening and/or possibly fatal. If such a
situation arises where a more serious issue is suspected by the foster, they are required to contact an
administrator as soon as possible and relay the issue in as much detail as possible. They will then be
advised as to how to proceed by one or both of the administrators. The Sweet Souls Foundation will pay
for testing up to a certain amount and then consult with the attending veterinarian for results and
prognosis. If further expense for treatment is required that exceeds the amount designated by the SSF
team for such treatment, the SSF Board will take the situation under review to determine further action and reasonable expense.
Should the situation be an instance of FIP, Pan leukopenia, advanced FeLV or
FIV or a combination of both or some other potentially fatal issue or a life-threatening issue that may
COMPROMISE THE QUALITY OF LIFE and the SSF Board determines humane and merciful euthanasia to
be a necessity, the foster is encouraged to comply with the Board’s decision. If the foster guardian
decides to disregard the Board’s decision and proceed with life-saving procedures, the expense of
further medical procedures will be the responsibility of the foster to fund in their entirety and SSF will
not be responsible for the repayment or reimbursement of them.
(initial) I understand the history of most foster animals is unknown and Sweet Souls Foundation
will not be responsible for any inadvertent exposure of foster parents or their own animals to unknown
disease or other medical condition. I knowingly accept this risk on behalf of myself and my own animals.
(initial) I understand that if I can no longer care for my foster , I must notify an administrator
immediately but that I must also anticipate it may take a minimum of two weeks to find an alternate
foster placement. Two week (14 days) notice to request an alternate placement is mandatory. If I fail to
provide two weeks’ notice to an administrator and an alternate placement has yet to be found, I will be
expected to pay for boarding of my foster at a suitable veterinary hospital for the remainder of the 14
day period I am not able to honor as a foster guardian as per this foster agreement.
(initial) I understand that any approved medical expenses that are paid by me, (out of pocket, instead of on the the rescues account) are considered a donation to the rescue and are tax deductible.
I understand I have 30 days to submit for reimbursement for any medical expenses that were approved but not placed on the rescue account. Once that 30 days has passed I will not be eligible for reimbursement.
(initial) I understand that if my foster escapes from the confinement, safety and care of my home
environment or the area where I am keeping the foster, I must notify a SSF administrator immediately.
(initial) I understand that my foster animal must be adopted out through Sweet Souls Foundation
adoption procedures. When interest is expressed by an adopter, they must fill out an adoption
application and this application must be submitted to a SSF administrator either by the foster guardian
or the interested adopter for approval. If approved, a Meet & Greet should be arranged between the
foster guardian and interested adopter. If all goes well, the foster and adopter can arrange a home
check at the adopters home prior to delivery of the animal or the home check can be done upon delivery
of the foster on the day the adoption is finalized - however, if the foster guardian should feel any
reservations at any point in time about the adopter, they are within their rights and highly encouraged
to abandon the adoption proceedings and notify a SSF administrator as to why they have opted to do so.
(initial) I understand that listing cats on Craigslist, eBay Classifieds or any media forum of this
nature is strictly prohibited. If the foster guardian wishes to share their adoptable listings on a Facebook
exchange site, the site must be approved by Kathleen Hom.
(initial) I understand the adoption fee for every SSF kitty is $55. Under NO circumstances are any
cats/kittens in foster care to be given away without express permission of a Sweet Souls Foundation
administrator Kathleen Hom or Sharyl McBride.
(initial) I understand there is no guarantee of an animal going to any specific individual even if
promised by the foster guardian. Each applicant must complete an application, be approved by an
administrator, attend a Meet & Greet/Foster Interview as well as submit to a home check inspection
before being allowed to adopt any animal under Sweet Souls Foundation foster care program. Payment
of adoption fees in full must also transpire before a foster should ever be left in the care of their new
(initial) I understand that Sweet Souls Foundation has the right to remove any animal from a
foster home at any time and for any reason.
(initial) I understand that each SSF foster guardian is allowed to be a "Foster Failure" and adopt
their foster pet at half the fee.
(initial) I authorize Sweet Souls Foundation to contact any person or entity named in this
application to verify information given. A Sweet Souls Foundation representative may visit my home
before this application is approved or during the time that I am fostering an animal for Sweet Souls
(initial) I understand that Sweet Souls Foundation will not be liable for any injury or damage to
persons or things caused by a foster animal in my care and I agree to indemnify and hold Sweet Souls
Foundation harmless from any such injury or damage.
(initial) I understand that all animals ultimately belong to Sweet Souls Foundation and if any
animal is miss-treated, abused, "goes missing" or is maliciously harmed by the guardian themselves or
while in the care of the foster guardian authorities will be contacted and legal action will be taken.
